• Completed Application for Admission Form (If providing a Centrelink or DVA Assessment of Assets letter, then Section B & C of this form do not need to be completed)
• Copy of the current ACCR (Aged Care Client Record)
• Copy of the Centrelink or DVA Assessment of Assets

Once the above documentation has been completed, please forward to the Manager of the facility to which you are applying.

Please note that a person will not be place on the waiting list until all of the above documents are provided.
