A Community Aged Care Package (CACP) supports frail older people living in the community who wish to remain living in their own home.

The package provides a planned and coordinated group of services to assist with complex needs. These people would also be eligible for low level residential care. Eligibility is assessed by the Aged Care Assessment Services (ACAS).

Strathdon community provides 100 CACPs for people living in the municipalities of Boroondara, Knox, Whitehorse and Monash.
