A Community Aged Care Package (CACP) supports frail older people living in the community who wish to remain living in their own home.

The package provides a planned and coordinated group of services to assist with complex needs. These people would also be eligible for low level residential care. Eligibility is assessed by the Aged Care Assessment Services (ACAS).

Aldersgate Community Programs provides 34 CACPs for people living in the municipalities of West Tamar, Meander Valley, Launceston & George Town.
