Through partnership with Eastern Health, Uniting Aged Care provides transition care to assist older people to recover following a hospital stay. 

Transition care can provide families and carers time to consider ongoing care arrangements.  This may include returning home with community support or accessing support within an aged care residence.

Whist in hospital Eastern Health staff can discuss your individual care needs with you and your family members.  If you are interested in receiving transition care, Eastern Health will arrange for you to be assessed by an Aged Care Assessment Team as someone eligible for transition care.   Access to Transition Care is available by referral through the Eastern Health Transition Care Program.

Uniting Aged Care currently provides 30 beds at Strathdon Community in Forest Hill, and will provide an additional 15 beds at Box Hill in agreement with Eastern Health by late 2009.
