Getting Started
If you think you could benefit from the services provided through a CACP, EACH or EACH-D package you will need to be assessed by your local Aged Care Assessment Service (ACAS) in Victoria or Aged Care Assessment Team (ACAT) in Tasmania, for eligibility. Assessment is free.

Your doctor can arrange an assessment for you or you may contact ACAS/ACAT directly by calling the Commonwealth Carelink Centre on 1800 052 222 to locate the ACAS/ACAT office nearest you.


Arranging Your Care Package
When a package is available, Uniting Aged Care will allocate a Case Manager to work with you, your family or your representative.

Your Case Manager will provide you with information to help you to choose the care services that best meet your individual needs and preferences.

Our Case Managers are a qualified team and will visit you, plan your care and follow up with you to ensure that you are satisfied with the services provided.


What If Your Care Needs Change?
Your Case Manager will continually assess, monitor and review your situation to ensure that your care needs are being met.

If your care needs increase and you require a greater level of assistance than you are able to receive through a Community Aged Care package, your Case Manager can talk to you about your options and the services that may be available to you.


Service Fees
You will be asked to make a contribution which is means tested and based on individual circumstances. If you receive a pension, you will not be required to pay an amount greater than 17.5% of your pension.

Uniting Aged Care does not refuse services to clients if they are unable to afford to make a contribution to their care provision.

Additional Note for Kalkee Community Care Clients:- The Case Manager in your area is available to discuss your payment options with you. Kalkee Community Programs in Geelong offer Community Aged Care Packages at a fee for service to people who require care but have not yet been assessed by their local Aged Care Assessment Service, or who have been assessed and await placement in residential aged care.
