Uniting Aged Care offers a wide range of community care services designed to help improve the health and wellbeing of older people and to support them to remain living in their own home. These services include:
Day Therapy Centres Our day therapy centres provide education, therapy and support designed specifically for older people to maintain or recover their independence. Services may include: physiotherapy, occupational therapy, speech pathology, social work, podiatry, nursing services, therapeutic massage/myotherapy, strength training, diversional therapy, group therapeutic activities and health promotion.
Care In Your Home
Care In Your Home is available through the provision of Community Aged Care Packages (CACPs), Extended Aged Care at Home packages (EACH) and Extended Aged Care at Home – Dementia specific packages (EACH-D). These programs offer assistance with a wide range of daily tasks including: personal care, house maintenance, short-term in-home respite, meals, shopping and banking, transport to appointments and social activities. Our fully qualified Case Managers make regular home visits, manage care plans and follow up to ensure clients are receiving the services they require.
Day Centres and Planned Activity Groups A variety of social and leisure activities give people a chance to socialise with others who have common interests, to dine out and/or to take a day trip. Some groups meet to participate in activities or to listen to guest speakers on topics of interest. Centre-based care is available for people living with dementia offering opportunities for small group social and leisure activities. Our ‘Well for Life’ programs provide an opportunity for exercise, strength training and health education. Most programs and activities include lunch and transport. Programs that bring carers together are also available.
Community Respite Services
These services offer respite for carers in the home, in day programs and small group outings or overnight in a residential facility. Aged Care Assessment is not required for most Community Respite programs.
