Eligibility
People eligible for our Planned Activity Groups:
- are older people
- people living with a disability
- people living with dementia and their carers.
Access
You can access the programs by contacting the Program Coordinator directly or by obtaining a referral from your doctor, family, friend or another service provider.
Service Fees
While all Planned Activity Group programs are funded through the Department of Human Services in Victoria and the Department of Health and Human Services in Tasmania, you will be asked to make only a small contribution. Your contribution is means tested and based on your individual circumstances.
Uniting Aged Care does not refuse services to clients if they are unable to afford to make a contribution to their care provision.
The Coordinator in your area is available to discuss your payment options with you.
Individual Needs
Care staff are available for clients requiring physical assistance.
Transport
Transport to and from our activities can be arranged. Please speak to the Program Coordinator who can coordinate the transport you may require.
Meals
Morning tea and lunch is included at most programs. You will be provided with information about what you might need to bring before you attend.
